What a support coordinator does
A support coordinator helps the participant understand and use their plan, connect with providers, organise how supports work together and build confidence managing their supports.
How they may communicate
With the participant’s consent, a support coordinator and service provider may communicate about service setup, goals, schedules or changes. Information sharing should be limited to what is needed and what the participant has agreed to share.
Which role do you need?
That depends on what is funded and what help the participant needs. Someone who needs hands-on assistance may use support workers. Someone with funded support coordination may use a coordinator to help organise and understand their wider support arrangements.
