NDIS Transition Support Planning Checklist
A good transition plan connects accommodation, disability supports, health responsibilities, equipment, transport and daily routines before the move happens. Use this checklist to identify gaps early.
1. Confirm the destination and timing
Record the expected move date, address, access arrangements, who is coordinating the transition and what still needs to happen before the destination is ready.
2. Map disability supports
List personal care, daily living, overnight support, community access, transport, communication and other supports the participant needs from the first day. Confirm which provider is responsible for each support.
3. Check equipment and accessibility
Confirm mobility equipment, hoists, bathroom access, home modifications, medication storage, assistive technology and any other disability-related requirements are ready before arrival.
4. Coordinate health and medication information
For hospital discharge or other complex transitions, clarify which health needs remain the responsibility of the health system and which disability supports will be provided through the NDIS plan. Share information only with consent and where it is needed.
5. Preserve routines and participant choice
Record communication preferences, meals, sleep, activities, cultural needs, preferred workers and important routines. The participant should be involved in decisions about how the transition is carried out.
6. Plan the first 72 hours
Make sure there is a clear contact person, worker coverage, transport, meals, medication processes, emergency contacts and a way to escalate practical problems after the move.
7. Schedule a follow-up
Set a review point after the move to ask the participant what is working, what is not working and what needs to change.

No. Hospital staff are responsible for clinical discharge planning. This checklist is a practical disability-support planning aid and does not replace health-system responsibilities.
Who should coordinate the transition?
It depends on the participant’s situation. The participant, nominee, provider, support coordinator where involved, NDIA contacts and health or housing teams may each have different responsibilities.
What is the most important thing to avoid?
Avoid a move where the participant arrives before essential support workers, equipment, access arrangements or medication processes are ready.
