MTA After Hospital Discharge
A participant who is medically ready to leave hospital may still need disability-related accommodation or home-and-living arrangements before they can move into a long-term home. MTA can sometimes help bridge that gap.
Hospital discharge planning comes first
Hospital staff plan for a safe discharge and may work with the NDIA, a Hospital Liaison Officer, the participant, family and a support coordinator where one is involved. Disability support needs after discharge should be identified before the move.
When MTA may be considered
Current NDIS guidance says a participant leaving hospital can have MTA added to their plan if they are eligible for SDA, SIL, ILO or home modifications. A confirmed long-term home is not required in this hospital-discharge pathway.
MTA does not replace health care
MTA is accommodation funding, not hospital treatment, nursing or rehabilitation. Health services remain responsible for health care, while NDIS supports address eligible disability-related needs.
Prepare the disability supports separately
Personal care, support-worker assistance, equipment, transport and other funded supports need to be organised separately from the MTA accommodation payment. Clarify who is responsible for each part before the discharge date.
Keep the long-term transition moving
MTA is temporary. The team should continue working on the longer-term home, modifications or support arrangement so the participant is not left without a clear exit pathway.

Do I need a confirmed long-term home before hospital discharge MTA?
Current NDIS guidance says participants leaving hospital who are eligible for SDA, SIL, ILO or home modifications do not need to have a confirmed long-term home before MTA can be considered.
Does MTA pay for nursing or personal care?
No. MTA funds temporary accommodation. Health care and disability supports are arranged and funded separately according to the participant’s circumstances and plan.
Who may be involved in discharge planning?
Hospital staff, NDIA staff such as a Hospital Liaison Officer, the participant, family or nominee, and a support coordinator where one is involved may all contribute to the transition plan.
