Preparing for a plan change

NDIS Plan Review and Reassessment Guide

People still commonly say “plan review”, but current NDIS information uses terms such as plan reassessment and plan variation. This guide helps you organise the practical information that may be useful before discussing a change to your plan.

What this guide covers

People still commonly say “plan review”, but current NDIS information uses terms such as plan reassessment and plan variation. This guide helps you organise the practical information that may be useful before discussing a change to your plan.

This page is general information. Your own NDIS plan, support needs and current NDIS rules determine what applies to you.

Start with what has changed

Write down what is different since the current plan was approved. Focus on changes in disability support needs, daily function, living arrangements, informal supports, safety, equipment, therapy recommendations or other circumstances that affect the supports you need.

Use specific examples. A short record of what happens in daily life is usually more useful than a general statement that “more support is needed”.

Organise evidence around support needs

Relevant evidence may include reports or letters from appropriately qualified health or allied health professionals, functional assessments, incident information, support records, progress reports or other documents that explain the change and its impact.

The evidence should connect the participant’s disability-related needs with the supports being requested. The NDIS may require particular forms or evidence depending on the type of change.

Review goals and current supports

Consider which supports are working, which are not being used, where there are gaps and whether the current mix of providers is helping you work towards your goals.

It can also help to review service agreements, support hours, transport arrangements and any upcoming changes such as moving home, starting work or study, leaving hospital or changing informal supports.

Know that more than one outcome is possible

Current NDIS guidance explains that a request may result in no reassessment, a plan reassessment, or a variation to the existing plan depending on the circumstances and evidence.

Because legislation, forms and processes can change, check the current NDIS plan-reassessment page before lodging a request rather than relying on an old checklist.

Official NDIS information used for this guide

NDIS legislation, terminology, funding rules and processes can change. Check the current official information for your circumstances.

Important: This AIC Services guide is general information, not an NDIS funding decision, clinical advice or legal advice.

Frequently asked questions

Is an NDIS plan review now called a plan reassessment?

“Plan review” is still widely used in everyday language, but current NDIS information uses terms including plan reassessment and plan variation. Check the current NDIS website for the process that applies to your situation.

What evidence should I prepare?

Prepare evidence that explains the disability-related change, how it affects daily life and why different or additional supports may be needed. The exact evidence and forms depend on the request and current NDIS requirements.

Can my provider guarantee that extra funding will be approved?

No. A provider can supply service information or support records where appropriate, but only the NDIA can decide whether a plan is reassessed, varied or funded differently.
