ILO Service Proposal Checklist
An ILO service proposal explains the participant’s preferred living arrangement and how the approved ILO supports will work in practice. It should be specific enough to show who is involved, how support will be provided and how the arrangement will be monitored.
Describe the preferred living arrangement
Document where the participant wants to live, who they want to live with, why the arrangement suits them and what an ordinary day and week should look like.
Define the primary support
Explain the host or housemate arrangement, the expected role of the primary support, how the relationship will work and what responsibilities are and are not included.
Map supplementary and back-up support
List paid and unpaid supplementary supports, planned absences, emergency contacts, on-call or drop-in arrangements and how support will continue if the primary arrangement is temporarily unavailable.
Set monitoring and problem-solving arrangements
Include how often the arrangement will be reviewed, who will participate, how concerns will be raised, how conflict will be addressed and what changes would trigger a formal review.
Check agreements, budget and implementation
Before submission, check that roles and payment arrangements are documented, the design fits the approved budget and the provider can practically implement the arrangement once approved.
